Patient Portal
myHealthPointe® is our safe, secure, and convenient patient portal, where you can gain access 24/7 to your health information. From this portal, you can view your health records with Outreach, request new appointments, learn more about your treatment options, see your medications and records, and much more.
If you’ve already used myHealthPointe® portal, all you need to do to get log on from here is to click this link https://myhpconsumer.netsmartcloud.com/Account/Login and enter your UserID (your email) and Password.
If you’re new to myHealthPointe® just follow the simple instructions below:
- Click on the portal link in the Welcome email sent from myHealthPointe® (aka: myhp@ntst.com) following your most recent visit.
- Once you’re on the portal, fill in the “Validation Code” field with the number provided in the email from myHealthPointe®.
- Check (✔️) the reCAPTCHA box.
- Click “Validate.”
- A new “Create Password” screen will appear. It will ask you to create your new Password.
- Once you have created your new Password, you are all set to log into the myHealthPointe® portal using your UserID (your email) and your new Password!
